NEW PATIENTS ARE VERY WELCOME AT BEECHES SURGERY!

Please complete a registration form (see below). Alternatively, our reception staff can supply you with forms and will be happy to guide you through the procedure. 

You may print off the registration form and Registration Questionnaire, fill them out and bring them in with you on your first visit to the practice.
